## Audit Summary
- Period: 2026-03-06 to 2026-10-26 (144 commits, 7+ months)
- Status: 🟡 YELLOW - New findings in OAuth and config validation
- New findings: 3 (#13 MEDIUM, #14 MEDIUM, #15 LOW)
- Total active findings: 11 (was 8)
- Security improvements: MCP auth, loopback binding, path-safety fix

## New Findings

**Finding #13 (MEDIUM):** OAuth credential file permissions not enforced
- container-manager.ts:336 - writeCredentialsFile() doesn't set mode 0600
- Regression check confirming #11 issue persists

**Finding #14 (MEDIUM):** Unvalidated settings config field
- schema.ts:1088 - z.unknown() accepts arbitrary SDK settings
- Unknown what settings are security-sensitive

**Finding #15 (LOW):** OAuth error logging may leak tokens
- container-manager.ts:369,389 - HTTP responses and errors logged unsanitized

## New Attack Surface

**File Serving Route:** /files/:agentName/* (APPROVED)
- Well-defended with realpath resolution, containment checks, TOCTOU prevention
- CSP sandbox headers mitigate untrusted agent content
- Same auth gap as web API (#12) - needs localhost-only documentation

## Hot Spot Analysis

Critical files verified (WARN status):
- container-manager.ts: OAuth credential handling modified
- container-runner.ts: MCP bridge integration
- config/schema.ts: New settings/plugins/mcp fields
- state/session.ts: Session adoption system

## Security Improvements Noted

✅ MCP HTTP bridge now requires bearer token authentication
✅ MCP bridge binds to loopback (127.0.0.1) by default
✅ Path traversal fix for Windows path separators
✅ Shell escaping improved ($, backtick, \, ")

## Priority Actions

1. HIGH: Document web dashboard + file serving as localhost-only (#12)
2. MEDIUM: Fix OAuth file permissions - add mode 0o600 (#13)
3. MEDIUM: Review SDK settings validation requirements (#14)
4. MEDIUM: Sanitize OAuth error logging (#15)

## Files Updated

Possibly related PRs

  • edspencer/herdctl#482: It updates the same changelog state and What's New page, and covers overlapping release-note content.

This PR only updates documentation and audit records, so it has no direct runtime effect. Some security audit statements are inaccurate or incomplete, notably the claim that the file-serving route has no race window and the credential-permission remediation that would leave existing files unchanged. The audit counts and statuses also disagree across files. Correct these before the records are relied on, but they do not block the release notes.

Ground Finding #014 in a threat model before keeping the MEDIUM rating.

The implementation only establishes that arbitrary configuration values can reach the SDK. The report names no untrusted producer, security-sensitive setting, or security-control bypass. If fleet config authors are trusted and the field is not user-facing, reclassify #014 as an open design review or advisory. Otherwise, name the untrusted boundary and the concrete setting and consequence.

Correct the TOCTOU assessment before approving the route.

realpath(resolved) runs before open(realPath, "r"). If an agent or other writer replaces realPath between those calls, open() can follow a new symlink outside the working directory. fstat() then validates the opened target, not the path that passed containment. Record this residual race and require an fd-relative, no-follow open strategy or equivalent race test.

Describe the symlink check accurately.

realpath() resolves symlinks, and stats.isFile() checks only the resolved target. A symlink to a regular file inside the working directory is served. The route rejects symlink escapes, not all symlinks.

Reconcile the finding totals across the audit artifacts.

This report says there are 8 active findings, but its severity rows total 7 because #008 is omitted. FINDINGS-INDEX.md lists #008 as an active Medium finding and counts 11 active entries because it includes accepted and intentional findings. Define the counting scope, then update both summaries and the final status line.

Make the permission remediation effective for existing files.

Reconcile the active-finding count.

The visible inventory lists three new findings and five carryover findings, so it accounts for eight findings. The severity rows also total eight, but Total Active Findings reports eleven. If three additional active findings exist, list them and their severities in this summary or the findings index. Otherwise change the total to eight.

Reconcile the count in the summary’s active-findings table: the listed severity
rows total eight, not eleven. Update “Total Active Findings” to eight unless
three additional active findings are verified; if they exist, include their
identifiers and severities in the summary or findings index.

`writeCredentialsFile()` does not set file mode to `0600` when writing OAuth credentials. On multi-user systems, this may expose tokens to other users.

**Recommended Fix:** Add `{ mode: 0o600 }` to `fs.writeFileSync()` call.

Make the permissions fix cover existing credential files.

{ mode: 0o600 } on fs.writeFileSync() only controls the mode when the file is created. The implementation reads an existing credentials file before writing it, so an existing permissive mode remains unchanged. Recommend enforcing 0600 on both create and update, for example with chmod or fchmod after opening the file.

Update the Recommended Fix guidance for fs.writeFileSync to specify enforcing
mode 0600 for both newly created and existing credentials files, noting that
chmod or fchmod is needed because the mode option only applies at creation.
